Our COVID-19 Guidelines
Our office will re-open once we may resume routine eye care. We have implemented protocols to provide a safe environment for excellent service. Our office staff members are screened daily and will always wear face masks and other protective equipment. We observe social distancing and schedule our entire office to be disinfected throughout the day. Exam rooms and diagnostic equipment will be disinfected before and after each patient.
To keep everyone safe, there are new guidelines for your visit. Please make an appointment on our website TheresaChew.com, on ZocDoc, or by phone (415) 677-9930.
Before Your Appointment
- Please fill out and submit the patient history form on our website.
- Please bring a face ask to wear at all times.
- Please note that only patients are allowed into the office unless assistance is required.
At Your Appointment
- Please plan to arrive 10 – 15 minutes before your appointment and call (415) 677-9930 when you arrive at our office.
- Our staff member will open the door to let you into the office, take your temperature with the non-contact thermometer, and give hand sanitizer to sanitize your hands.
- You will answer some questions pertinent to COVID-19. This is in accordance with the City and County of San Francisco Department of Public Health (SFDPH) and the Centers for Disease Control & Prevention (CDC).
- Please maintain social distancing throughout your visit.
Our Dispensary and Frame Selection
- All of our frames are sanitized thoroughly with a UV sanitizer, hydrogen peroxide, plus soap and water.
- Our staff member will bring you a tray of frames for selection.
- After trying out, please put in tray to be sanitized by our staff member before they are returned to the frame board.
